It may seem a little overwhelming but the reality is if you take the 30 minutes to completely fill out the information it will make creating a direction to target recovering your health so much easier!! We encourage your spouse or significant other to attend the initial consultation so they can be supportive in your health. During the initial visit we will be consulting to see if your case qualifies in our office, reviewing your health history, assessment forms, Life impact forms, performing a neurologic screening, and possibly choosing some additional lab test to be run to further investigate your health concerns. After this initial visit you will be schedule within 10 days for a follow up case review and recommendations. Please allow 45 minutes each for the first two visits.
